He plays the violin and is being taught by a mute homeless man who only speaks through musical notes on his own violin.

You must memorize the notes then sing them back to the blind man, who translates it to basically, “you have problems with music because you cannot hear properly. “Make your way to the Doctor.”

Because it is written as “Make your way to the doctor”, he translates it as “you got to do this on your own”.

So he asks you to find a suitable and safe path, then pee on everything (while transformed as a dog) because he has already memorized your pee scent. How? We’ll never know.
